Employees who are enrolled in the State of Tennessee and Higher Education Health Insurance can earn $50<\/strong> for completing a biometric screening!<\/em><\/p>\n

When and Where<\/strong>:<\/p>\n

July 14, August 5 & 6 8:30-10:30<\/p>\n

University Health Services<\/p>\n

This screening is by appointment. There is no employee access fee for this service.\u00a0\u00a0 Lab billing will be filed through your insurance plan-please bring your insurance information to your appointment.\u00a0 For questions related to this, call 425-2266.<\/p>\n

Register for an appointment:<\/strong><\/p>\n

Use the following link to access your medicat patient portal: https:\/\/utc.medicatconnect.com\/home.aspx<\/a><\/u><\/strong><\/p>\n

  1. Login with your UTC login<\/li>\n
  2. Select \u2018Appointments\u2019 and follow the instructions listed<\/li>\n<\/ol>\n

    If these times do not work for your schedule or the appointments are full, you may contact UHS at 425-2266 for an appointment.\u00a0<\/em>\u00a0<\/strong><\/p>\n

    Get Ready for Screening \u2013 follow these steps:<\/strong><\/p>\n

    1<\/strong>– Download and print the Physicians Results Form.<\/p>\n

    How to Download and Upload a Physician Results Form<\/a><\/u><\/p>\n

    *You must take your real age assessment for this form to be printed*<\/em><\/strong><\/p>\n

    If you need assistance with this process, call 1-855-623-9355.\u00a0<\/em><\/p>\n

    2<\/strong>-Please fast for this screening. Do not eat after midnight the night before your appointment.\u00a0 You may have water, black coffee and routine medications during your fasting period.\u00a0 We encourage breakfast or a snack after<\/strong> your appointment.<\/p>\n

    3<\/strong>-Bring the Physicians Results Form and State of Tennessee and Higher Education insurance card to your appointment. We will measure your height\/weight, vital signs and draw blood for labs.<\/p>\n

    Get Lab Results and Cash! <\/strong>When your lab results are back, UHS staff will complete your form and fax it to Quest Diagnostics at 1-844-560-5221. If any of the lab results are out of normal range, UHS will send you a secure message through the Medicat patient portal.<\/p>\n

    What about other Cash Incentives? <\/strong>Visit the UT Wellness Program-State and Higher Education<\/a> website and activate your wellness program account to submit documentation of wellness activities.
