If you are enrolled in State of Tennessee and Higher Education Health Insurance, you can earn\u00a0$50\u00a0for completing a biometric screening!<\/em>
When and Where<\/strong>:
Monday, April 17 and Tuesday, April 18
8:00 am – 10:30 am
Chattanooga Room A, University Center\u00a0\u00a0<\/p>\n\n\n\n

This screening is by appointment and at NO COST when you bring your insurance information to your appointment. After your screening, have a light snack and enjoy our health education resources. For questions related to this process, call 425-2260.  <\/p>\n\n\n\n

Register for an appointment!<\/a> <\/strong>If these times do not work for your schedule or the appointments are full, you may contact UHS at 425-2266 for an appointment. <\/em> <\/strong> <\/p>\n\n\n\n

Get Ready for the Screening \u2013 follow these steps 1, 2, and 3!<\/strong>  <\/p>\n\n\n\n

    \n
  1. Go to https:\/\/my.questforhealth.com<\/a><\/strong> and register or login. The registration key is SOT; your unique ID is your Edison ID which is found on your Caremark card. Download and print the Physicians Results Form.  If you are unable to register online or need assistance with this process, call 1-855-623-9355. <\/em>  <\/li>\n\n\n\n
  2. The night before your appointment, do not eat after midnight, but drink plenty of water. Take any regularly scheduled medications as prescribed by your healthcare provider.  <\/li>\n\n\n\n
  3. Bring the Physicians Results Form and State of Tennessee and Higher Education insurance card to your appointment. We will measure your height and weight, take your blood pressure and draw blood for labs.  <\/li>\n<\/ol>\n\n\n\n

    Get Lab Results and Cash! <\/strong>When your lab results are back, UHS staff will complete your form and fax it to Quest Diagnostics at 1-844-560-5221.If any of the lab results are out of normal range, UHS will send you a secure message through the Medicat patient portal. <\/p>\n\n\n\n

    What about other Cash Incentives? <\/strong>Visit the UT Wellness Program-State and Higher Education<\/a><\/strong> website and activate your wellness program account to submit documentation of wellness activities. The 2023 Wellness Program Incentive Table<\/a><\/strong> describes other wellness activities that can help you earn cash.  <\/p>\n\n\n\n

    Public Health Survey:<\/strong> Want to give your input on health concerns and health issues as a Hamilton County, Tennessee resident? UTC and the Hamilton County Health Department are partnering on a Public Health Survey. This is an anonymous survey to help us know what programs are needed in Hamilton County.
