data-contrast=\"auto\">background check is required for most facilities. Many sites offer national background checks. Different sites will have various costs based on the insurance the user holds. One website\u00a0<\/span><span data-contrast=\"auto\">our\u00a0<\/span><span data-contrast=\"auto\">students<\/span><span data-contrast=\"auto\"> often use is <a href=\"https:\/\/www.backgroundchecks.com\/personal\/criminal-background-check\">backgroundchecks.com<\/a><\/span><span data-contrast=\"auto\">. To begin the process, follow the link and begin to fill out the form titled &#8220;Start Your Check Today.&#8221;\u00a0 Then, follow the prompts. Make sure to fill out the <strong>personal<\/strong> background check.<\/span><\/p>\n<p><span style=\"text-decoration: underline\"><b>Immunization Record<\/b>\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">A certificate of immunizations can be received from the TN Department of Health.<\/span><span data-contrast=\"auto\">\u00a0Clinic locations are located on the\u00a0<\/span><span data-contrast=\"auto\">TDOH Hamilton County<\/span><span data-contrast=\"auto\">\u00a0<\/span><a href=\"http:\/\/health.hamiltontn.org\/\"><span data-contrast=\"none\">website<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-contrast=\"auto\">\u00a0<\/span><span data-contrast=\"auto\">Print out the attached\u00a0<\/span><a href=\"https:\/\/new.utc.edu\/sites\/default\/files\/2021-03\/CPE-Immunization%20Record%20Form_0.pdf\"><span data-contrast=\"auto\">PDF<\/span><\/a><span data-contrast=\"auto\">\u00a0and ask for your records\u00a0<\/span><span data-contrast=\"auto\">at the location.<\/span><span data-contrast=\"auto\">\u00a0<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">NOTE: Immunization records may not\u00a0<\/span><span data-contrast=\"auto\">be located in<\/span><span data-contrast=\"auto\">\u00a0your current area of residence but instead may be located where you\u00a0<\/span><span data-contrast=\"auto\">were born<\/span><span data-contrast=\"auto\">\u00a0(as most records are from one\u2019s childhood). For example, if you\u00a0<\/span><span data-contrast=\"auto\">were born<\/span><span data-contrast=\"auto\">\u00a0in Wisconsin, your records may\u00a0<\/span><span data-contrast=\"auto\">be located\u00a0<\/span><span data-contrast=\"auto\">in<\/span><span data-contrast=\"auto\">\u00a0Wisconsin<\/span><span data-contrast=\"auto\">. You will need to contact the\u00a0<\/span><span data-contrast=\"auto\">state\u00a0<\/span><span data-contrast=\"auto\">department that currently holds your records<\/span><span data-contrast=\"auto\">, and they should be able to email or mail you a copy of your records.<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559739&quot;:160,&quot;335559740&quot;:259}\">\u00a0<\/span><\/p>\n<p><i>It should be noted that the following 3 items (Drug Panel Screening, PPD <\/i><i>[<\/i><i>TB<\/i><i>]<\/i><i>\u00a0Test Results, and the Physician Statement of Health) can all be obtained in the same location. Clinics such as American Family Care Urgent Care will complete all 3 items for you, and there are<\/i><i>\u00a0<\/i><i>several<\/i><i>\u00a0o<\/i><i>ffices<\/i><i>\u00a0throughout<\/i><i> the Chattanooga Region.<\/i><\/p>\n<p><span style=\"text-decoration: underline\"><b>Drug Panel Screening<\/b>\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Drug Panel Screenings can take place at most any clinic. Our other medical courses do not provide CPR training. That being said, depending on where you are placed for clinicals, your employer may still require CPR certification.
